It’s a solution :)

I installed this over the weekend and included the steering wheel control adapter.

The MVA-81 is doing a fair job. It’s is hard for people on the phone to hear me when I’m in the GX470 SUV - but it does work.
Significantly, I can use the Bluetooth on the radio to listen to podcasts, music and telephone.

Nothing about the steering control adapter works (pity). But overall satisfactory set up.

Before this I used a BT-45 adapter, but there were no radio controls in that set up.

Regarding the steering wheel control adapter—not all aftermarket head units support steering wheel button integration out of the box. Be sure to check that your head unit has the appropriate input or interface to enable this feature.
